Replacement Membership Certificate

Please Note: When making your order please fill out the required fields in order to verify membership.

Lifetime national membership dues. Membership includes a certificate and one-year subscription to the honor society's quarterly journal, The Historian. Enter the number of members you will be submitting. After making the payment, email your list to info@phialphatheta.org. When planning the date for your initiation ceremony, keep in mind the national office requires a 3-week turnaround time to process information before mailing the certificates to you.
